Yearly Required Forms for 4-H Clubs
Yearly Financial Summary Statement: This form is required to be completed annually by all clubs and 4-H affiliates. Forms are due to the county Extension office by January 31st. All income and expenses from the club must be tracked on this document. These forms are used for annual 990N submissions with the IRS to maintain non-profit and tax exempt status. Clubs may be selected for random audits to ensure adequate handling of funds.

Ohio 4-H Club Charter Check List: This form is required each year to maintain your 4-H Club charter. Forms are due to the county Extension office by March 15th.

Club Activity Fee Form: All members must pay dues for 4-H participation to the Madison County 4-H Advisory Committee. This form and payment are due after enrollment by March 31st to the 4-H Advisory Committee (mailed to their PO box, ONE club check only made out to the Madison County 4-H Committee). You must list each member enrolled and it should match the enrollment roster for your club. The fee is $15/member with a $30 cap for each family (cloverbuds and volunteers do not pay fees). Club Fund Raising request form: This form is due at least 30 days prior to beginning a fundraiser for your club. This short form is for the volunteer or club officer to complete prior to promoting and starting a fundraiser to ensure the fundraiser follows OSU and Ohio 4-H guidelines and the club has a need for raising funds.

4-H Club Constitution Template: All clubs must have a current copy of their Constitution on file at the county Extension Office. If changes are made, or if you start a new club, this form must be completed again and sent to the Extension Office for your club file.

4-H Club By-Laws Template: This form is optional, but highly encouraged for all clubs to use. A copy of the current by-laws must be on file at the county Extension office. By-laws can be in any format - this form is simply a template to assist you in your planning. By-laws include details such as meeting expectations, officers, project book guidelines, meeting attendance requirements, etc. This should be reviewed annually.

If your club is hosting a fundraiser where food is involved, or planning to serve food at a meeting, club activity, or service event, the volunteer in charge and present must have a current Occassional Quantity Cooks certification. This training can be completed online and the certification lasts for 3 years. For details, visit: Occasional Quantity Cooks Training (Directions) (Link to training).
Depending on the event, you may need to contact the Madison County Health Department to see if you need a Temporary Food Service License (details at Temporary Food Service Operations Guidelines and Application)
